When I found out the permit had been approved, I called for inspection for framing, mechanical and electrical. Framing failed (of course) but my work passes. However.. There's a problem with the permit though. It's the wrong kind. It's a counter permit that's used for an "In kind" fireplace replacement, not an installation requiring significant structural, electrical and plumbing changes. The inspector said he needs framing plans, load calculations, electrical and plumbing changes documented etc. The contractor emailed the inspector and told him something to the effect of "...we don't want or need another individual involved."

After ignoring the contractor for over a week, I agreed to allow him in to make "corrections". They pulled all the steel off, did something and put it back up. This is the start of the results. They actually made it worse!
