This is where it started. The fireplace is too small for the wall it's on, it's off center by over 2 inches to the left and the stone installation was pretty poorly done. Around the edges of the fireplace, you can see holes in the mortar and the metal lath itself. Definitely not up to my standards. Time for an upgrade.
